How to Request a Patient Contact List

Patient contact lists may be requested for outreach purposes, such as physician departure letters, newsletters, educational materials, or community outreach. To request a patient contact list, follow the steps below:

1. Submit the Request Form

2. Include All Required Information in the Request

  • Make sure to provide:
    • Reason for the request - briefly explain the purpose of the outreach (e.g., departure letter, newsletter, educational material, community outreach, etc.).
    • Inclusion/Exclusion criteria - list the criteria needed to identify the appropriate patient population.
    • Examples include:
      • Provider
      • Date of service range
      • Service location
    • Fields to include in the output - specify the fields/columns you want included in the contact list.
3. Submit Outreach Materials for Privacy Review 
  • Before any patient contact information can be released and to ensure patient privacy requirements are met, all outreach materials must be:
    • Sent to the Privacy Office
    • Reviewed and
    • Approved
